Your working checklist

Ask these questions in your trial. Open a question to see the evidence to keep; use the CSV to record your answers.

  1. BranchingDoes each route collect the intended answers, including after going back?

Ask for: Branch map and sample submissions.

  2. ValidationCan a visitor understand and correct missing or invalid answers?

Ask for: Error and recovery task.

  3. AccessibilityCan the intended visitor finish the embedded form using their input method?

Ask for: Keyboard, mobile and assistive-technology review.

  4. ConfirmationCan a submitted response be traced to its acknowledgement and destination?

Ask for: Response identifier and receipt.

  5. IntegrationHow do staff detect and recover a failed handover without duplicate records?

Ask for: Paused connection and retry trial.

  6. AccessWho can read responses, attachments and shared reports?

    Ask for: Role-by-route access test.

  7. ExportCan another person reconstruct answers, dates and attachments from the export?

    Ask for: Independent export read-back.

  8. LimitsWhich response, storage, collaborator and integration limits affect our plan?

Ask for: Quote for the expected and peak workload.

Work through the review

Build one short form using fictional information: a contact field, a choice that changes the next question, a required field and an optional attachment. Write down the expected response before submitting it. Repeat as a mobile visitor and a keyboard user, then follow the response into the notification, connected system and export. Use the same tasks in each product so attractive templates do not decide the comparison.

Test every route through the form

List the possible branches before configuring them. Submit a response through each route, then go back and change the answer that controls a branch. Inspect whether an answer to a now-hidden question remains in the submission. Repeat with an empty required field, a long response and an attachment at the configured limit. For scored surveys or calculated fields, retain the expected calculation and compare it with the exported value. Record the form version used so later edits do not make the trial impossible to reproduce.

Complete the task without a pointer

Use the keyboard to reach every field, select an option, correct an error and submit. Confirm that labels and instructions remain visible when the form is embedded on the intended page. Ask a reviewer using assistive technology to assess the real task. The W3C forms tutorial covers labels, grouping, instructions, validation and feedback; use these topics to structure the review. A template description or a vendor accessibility statement does not replace testing the configuration your visitors will actually encounter.

Follow one submission through a failed connection

Give each fictional response an identifier. Verify the confirmation, notification and destination record against that identifier. Pause a trial integration, submit a response and inspect how the failure is shown, who can see it and how the response is retried. Check for duplicate records after retrying. Repeat with the ordinary staff role instead of an administrator. Ask which features require another service or a higher plan. Keep a successful submission and a recovered failure as separate trial records.

Check the response boundary and the export

Define which staff should see responses, attachments and shared reports, then test those paths with the corresponding accounts. Use only fictional data. Export a sample with multiple-choice answers, Unicode text, an attachment and a submission timestamp. Read it in an independent spreadsheet and confirm whether column names, timezone, field identifiers and attachment links are understandable. Ask what happens after a field is renamed or deleted. Include response limits, storage, collaborator roles and integrations in the written quote for your expected workload.

Source and scope

The W3C tutorial explains accessible form labels, groups, instructions, validation and user feedback. The branching, integration and export tasks are our editorial purchasing exercises. This checklist does not certify a form or a platform as accessible.
